All requests for food are by APPOINTMENT ONLY and the following required documentation is necessary:1.  Picture ID needed for person filling out forms and all others need Drivers License, Medicaid Card, Social Service ID, Birth certiciates or School ID acceptable for children.2.  Verification of address (Lease/Electric Bill, etc.)
