The Chronic Renal Disease Program (CRDP) is administred by the Division of Medicaid and Medical Assistance (DMMA). The program is 100% state funded. All third party resources (Medicare, Medicaid, VA, Private insurance) must be considered before CRDP funds are used. The applicant must meet certain medical nad financial criteria in order to be eligible for funding from the CRDP. Medically, the individual must be diagnosed with End Stage Renal Disease (ESRD), receive dialysis or have had a renal transplant. Financially, the individual’s gross countable income must be below 300% of the Federal Poverty Level. Services provided by the CRDP can consist of payment for medication (prescriptions and over-the-counter), nutrional supplements, transportation and payment of Medicare part D costs.
