Provides case management of individuals and the families of those who are diagnosed with HIV/AIDS. Works with Medicaid waiver to help maintain clients with AIDS in the community. May assist the client to seek available limited emergency financial assistance. Eligibility determination for services and advocacy for persons with HIV/AIDS. Provides information and makes referrals to community resources.
