Community Services (DSAAPD) is the basic intake number for the Division of Services for Aging and Adults with Physical Disabilities. The Community Services program develops care plans for the evaluation and treatment of senior citizens and adults with physical disabilities who are unable to arrange services on their own behalf. Community Services assesses needs, coordinates the delivery of services, monitors progress and provides eligibility information for Homemaker Services, Adult Foster Care, Attendant Services, Personal Care, housekeeping, Emergency Response System, Assisted Living, general case management and the Elderly and Disabled Medicaid Waiver Program. Call for information about receiving copies of the Living Will. Community Services (DSAAPD) also administers the Home Modification Program. This program provides limited financial assistance to help modify homes so that eligible adults age 60 and over or disabled adults may live independently.
