Someford Place, is a Five Star Senior Living Community. It is an assisted living facility specifically for memory impaired residents, located next to Somerford House. This is a secured facility that serves 52 Alzheimer’s and dementia patients. All meals, services, activities, and outings are provided for residents. Fees at Somerford Place are based on the accommodations selected and the service plan needed. Detailed fees are listed above. No lease is required, but a 30 day notice must be given prior to a resident leaving. Service Plans are in five levels: Level I – is for those who are able to complete their own personal care; Level II – is for those who need reminders or minimal assistance with personal care such as bathing, dressing, and grooming; Level III – is for those who need a moderate amount of assistance to complete their activities of daily living; Level IV – is for those who need extensive assistance with personal care, transferring from bed, toilet or bath; and Level V – is extensive assistance including hospice and palliative care services.
